  • To the best of my knowledge, all the information provided on this form is correct. I understand that Miracle Hill Ministries carefully screens volunteers that interact with children through S.L.E.D./D.S.S. background checks and childrens specific orientation and I agree to the screening process. I also understand that if I choose to have direct interaction with guests I will be required to abide by the standards, policies, and procedures set forth by Miracle Hill Ministries's Volunteer Handbook* and the department in which I work. *volunteer handbook is available on MHM website
  • VOLUNTEER RELEASE OF ALL LIABILITY AND WAIVER Miracle Hill Ministries, Inc., (“Miracle Hill”) is a non-profit corporation organized and existing under the laws of the state of South Carolina. The individual indicated on page three (3) desires to provide volunteer services for Miracle Hill and engage in related volunteer activities. The volunteer understands that the scope of his/her relationship with Miracle Hill is limited to a volunteer position and that no compensation or benefits associated will be paid to the volunteer. In addition, the volunteer is responsible for his/her own insurance coverage in the event of personal injury or illness resulting from his/her service to Miracle Hill. RELEASE OF ALL LIABILITY/ASSUMPTION OF RISK: I wish to participate as a volunteer in the programs of Miracle Hill. I understand that there is the possibility that I could be injured as a result of participation in these programs. Nevertheless, in consideration for allowing me to volunteer in the programs, I agree that I, my assignees, heirs, spouses, guardians and legal representatives will not make a claim against, sue, or attach the property of Miracle Hill, its board members, officers, agents or employees, and any other persons, agencies, firms or corporations affiliated with Miracle Hill. I do hereby fully and completely release Miracle Hill from any and all actions, claims or demands that I, my assignees, heirs, spouses, guardians, and legal representatives now have or may hereafter have from any liability, whether currently known or unknown, including, but not limited to loss of life, personal injury and/or damage to property, resulting from, arising out of, or in any way connected with my volunteer participation. This release of liability and assumption of risk, in addition to covering any past occurrences, is intended to release and discharge in advance Miracle Hill, its board members, officers, agents or employees, and their successors and assigns from and against any and all liability arising out of or connected in any way with my participation in said programs even though that liability may arise out of negligence or carelessness on the part of Miracle Hill. INSURANCE: I understand that Miracle Hill does not assume any responsibility for or obligation to provide me with financial or other assistance including but not limited to medical, health or disability benefits or insurance in the event of my injury, illness, death or damage to property. If I voluntarily transport a Miracle Hill program participant in my personal vehicle, I agree to assume all risk and liability of doing so. I understand therefore, that I must possess automobile liability insurance of $100,000 per person, $300,000 per accident and $100,000 for property damage. PHOTOGRAPHY/VIDEOGRAPHY RELEASE: I hereby grant Miracle Hill permission to use my likeness in a photograph, video, or other digital media (all of which is referred to hereinafter as “photo”) in any and all of its publications, including web-based publications, without payment or other consideration. I understand and agree that all photos will become the property of Miracle Hill. I hereby irrevocably authorize Miracle Hill to edit, alter, copy, exhibit, publish, or distribute these photos for any lawful purpose. In addition, I waive any right to inspect or approve the finished product wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of the photo. I hereby hold harmless, release, and forever discharge Miracle Hill from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization. MIRACLE HILL MINISTRIES CODE OF ETHICS POLICY All volunteers associated with MHM are expected to comply with the Code of Ethics to ensure that ethical decision making and conduct are observed in all aspects of the ministry. This document should be viewed in conjunction with the relevant policies and procedures and should not be considered all inclusive. Confidentiality: All volunteers will protect the confidentiality of individuals receiving services through any MHM facility or program. Information concerning guests/clients and/or their treatment will not be revealed or released without the written consent of the guest/client. •Except as directed by MHM paid staff, volunteers will not reveal the identity of any guest/client or any other Personally Identifiable Information (PII) to anyone. PII is information that can be used on its own or with other information to identify, contact or locate a guest/client. This may include name, address, email address, date of birth, birthplace, etc. •Volunteers will abide by any directions of MHM paid staff concerning the disclosure of any other information of a personal nature relating to a guest / client. Gratuities/Influence: No volunteer affiliated with MHM will accept gifts of material value, favors or remuneration for personal gain from any individual, agency or organization that does business with MHM. Additionally, no volunteer will attempt to influence decisions of any funding source through donations of cash, promises of special consideration or suggestions of any valuable contributions. Preferential Treatment: MHM prohibits the preferential treatment of any volunteer applying for and receiving services. Services will be granted in order received using the necessary screening tools to ensure beneficial outcomes. External Relationships: For Adult Ministries, MHM prohibits any intimate or romantic relationships between volunteers and guests for one (1) year following the departure of the guest/client from the care of MHM, unless the relationship existed prior to the intake of the guest/client. For Children’s Ministry, MHM prohibits any intimate or romantic relationships between volunteers with clients. Required Reporting: Volunteers who observe employees or volunteers behaving in an inappropriate or unprofessional manner in the course of their assigned duties should report such behavior to the appropriate supervisor. Social Media Policy: All online media has a direct reflection on our ministry. Any information presented online to represent MHM, including all affiliated programs and facilities must reflect the belief, image, brand and mission of MHM. •Volunteers are not permitted to post any identifiable information related to the guests/clients residing within MHM facilities on social media. This includes, but is not limited to, photographs (including those with images placed over the individual’s face), silhouettes of the individual or the name or birth date of the individual. •Volunteers are required to refrain from participating in one-on-one social media relationships (e.g. Facebook friends, Twitter followers) with current guests/clients on social networking sites and may only interact with guests/clients through official MHM sites. •Any postings which identify the individual as a volunteer should not undermine MHM’s Code of Ethics, Mission and Values. •Volunteers posting content about MHM business on a personal social media account or site should include the following disclaimer: While I am connected as a volunteer with Miracle Hill Ministries, Inc., everything listed here is personal opinion. Opinions, conclusions and other information expressed here do not necessarily reflect the views of MHM. I hereby certify that I have read and understand this Release of All Liability, Assumption of Risk and Waiver and Code of Ethics Policy and that I have received a copy.
